I just got an email from UF saying to send in the supp app. Is it necessary to send the research form with the supp app. I have over 400 hours of neurology research however my research professor wont be reachable till mid sept. I want to submit my supp app as soon as possible. What do you guys think?

For the first part of the supp app, it says to input person claiming florida residency (i am an instate resident)...I am a dependent person so would I put my fathers name or mine? and should put his drivers license and car info or mine?
