Hey guys,
For those of you filling out the UF supplementary application, are you going to give the dental observation form to all of the dentists you shadowed? Are they supposed to fill it out and send it to the address themselves or should they fill it out, give us back, and we send all the forms as one??????????????

I called and asked, I was told that they can fill it out and give it back to us.
