I come from a state MD school in southeast with a step score of 240. I feel like I am average in everything else especially grades wise and not much research. I am uncertain on whether to do an AI at my first-choice (mid-tier school). I been reading mixed reviews that it will hurt you in IM. I don't know what to do and would like some advise. Thanks so much.

Dm me after match day and I can give you some advice. I didnt do an AI..got a bunch of interviews from pretty good places..not sure how itll pan out but from a low tier school in SE.

what region is your first-choice in? heard they're not usually beneficial unless you want to go to a competitive location with no ties i.e. NYC, CA

I want to stay in the southeast region.

Pretty sure you are a shoe-in for most IM programs as a state MD with 240 step scores and solid letters. Maybe look to see what sort of EMR this AI has and if you are familiar with it, then it would be worth a shot doing one there if you feel comfortable and confident that you could hold your own with the team.

I ended up doing an AI at another program here in town and I was lucky enough to not have any in-house medical students on my particular service (only me and two IMGs) which I think allowed me to shine a bit, on top of the fact that I was used to the EMR.

For internal medicine, an AI is usually not necessary unless you're really gunning for a place and want to schmooze with the PD. It can hurt you if you do poorly or clash personality-wise with the residents/attendings/PD.

Does your school have an IM program? If so, is it academic, community, or county? Did you or will you rotate at said IM program?

I don’t know enough about IM to advise you, but I’m sure the above information will help future people advise you.

Do people mean 'aways' when they're saying 'AI' in this thread?

An away is an AI done at an institution other than your home institution. An AI is a 4th year rotation in your desired specialty which is where most people get their LORs from.

I did no aways, but 3 AIs (General surgery doesn't require aways/audition rotations). Most of my classmates doing IM did not do aways, though they did multiple AIs.
