If we were in a combined degree MD/PhD program, do we just choose this, or also state that we've already obtained the PhD degree separately? My PhD is done and it's just the MD that's pending.

Also, should we state that we had a gap in medical education, or can we consider the PhD as part of an integrated medical training program? I'm inclined to do the latter and not state that there was any gap...
