Unmatched this year and wondering what the process is like for reapplying AOA/ACGME. Scheduled to start a TRI this year. Slightly confused on the whole process.

Questions:
1. Say I want to apply anesthesia and IM. Is the process for applying as a post-graduate the same as it is for a medical student? Do I have to take a year off between matching and starting? Or can I jump right into PGY-2 in 2016?

2. Will I receive some credit for the TRI I am doing now? Or will I have to repeat the year for anesthesia and IM?

3. I understand ACGME has special spots for physicians labeled "R", but can we also apply to the categorical/advanced spots? Is it harder to get a spot as a post-graduate in anesthesia/IM?
 
1. Yes, the process is the same. Do you need to take a year off? It depends -- if you match into a position starting in July 2016, then no. If you match into an advanced position starting July 2017, then yes. And you can apply for a PGY-2 spot in 2016 either via the match (R positions), or try to find one outside the match (like an IM PGY-2)

2. Maybe. If you continue in an AOA program, you might get some or all credit for your TRI. If you continue in an ACGME program, you will receive (in general) very little credit.

3. You only qualify for an R position if your PGY-1 "counts" towards your specialty. I don't know if a TRI is allowed for an ACGME anesthesia program. If you go to the American Board of Anesthesia website and check, they should list the requirements. But, in any case, you can apply for any combination of C, A, P, or R programs you would like.
