What is the most common causative organism of osteomyelitis in a sickle cell patient?

A) Staph aureus
B) Salmonella

My dillemma stems from this,

Goljan states that Salmonella is the most common cause of osteomyelitis in a HbS patient (he also says this has been asked on boards before)

Step-up states that though HbS patients are more susceptible to Salmonella, Staph aureus remains the most common cause of osteomyelitis.

Robbins avoids the subject with HbS pts are "predisposed to Salmonella infections".

BRS says "Salmonella is frequent in association with sickle cell disease".

First Aid says "Sickle Cell Osteomyelitis - Salmonella".

I'm leaning toward Answer B - What do you guys think?

That is one of the 150 released questions. And the answer is Salmonella. Most common overall is different from a single population.

Come on.... everyone knows the most common illness for HbS patients is Malaria 😀

My pathology professor said it's Staph. Aureus . He even emphasized it. Oh well, let's just hope we won't have this question.

What ya gotta realize about these tests are that they are asking each and every question for a reason. They want to test a specific concept. If they want to test S. aureus, they will do it w/a 12 y/o girl who is having knee pain or some random vignette like that. Whenever they ask about a pt w/sickle cell they want you to say salmonella.

In all fairness, if they want to test the Salmonella-sickle cell connection, the question should have Salmonella in the stem and sickle-cell disease as one of the answer choices. That would eliminate all the confusion.

Thanks for all of the input. I agree with everyone who has posted here & basically I just needed some conformation before I went against a text.

Thanks

The answer is indeed salmonella....it accounts for 70 % of cases of osteomyelitis in sickle cell anemia patients...comapred to the 25% due to staph aureus.....source usmleworld

just had the same question on UW. I'm going with Conrad on this age old question. The man said "S. Aureus is the MCC of osteomyleitis in all populations." This is much like then pneumonia- klebsiella- alcoholic question..#1 cause is still strep pneumo.
