is There a way to create blocks of questions for the entire topic (e.g. surgery)? For example, can I do all of the 136 surgery Qbank questions in blocks of 40? Right now I can only finish 40 questions and create another block and the questions are all randomized when I start new test.

Just click on surgery questions and hit unused questions and just create blocks of 40 with the filter being checked off for surgery and unused questions that way you'll finish all the surgery questions.
