MCV (VCU) is a very solid program based mainly on clinical experience. As mid-tier univeristy programs go, it is a very good program.
The program director is really great, very energetic and interested in making the program all it can be. The patient population is indigent which means you get to see the natural progression of disease.
The VA is a large, excellent VA and MCV hospital is a large, tertiary care center that takes care of roughly 50% of all indigent patients in Virginia. The research is there but it is not going to be the same kind of experience as a larger name program. All in all, I think it is a very good training program which will give you good options for fellowship. Obviously a name like MGH is going to go farther but you will get good training at MCV. I went to med school there and their medicine department is one of the reasons I chose to go into medicine.
