1) which places have the highest volume? am i right in assuming that hospitals with the highest volume means more operative experience for their residents?

2) ive noticed some programs are six years, some are seven. those programs that are 7 - most research year 6 before becoming chief in year 7. is it possible to do an infolded fellowship year 6 instead of research and then becoming chief in year 7, or is this dependent upon the program? which programs do you know allow this infolded fellowship option?
 
High volume doesn't allow mean more operative exerpience because it also depends on number of residents. The mix of cases within that volume is also important. Some places are weak in peripheral. At others it is peds or functional.

Some places allow an infolded fellowship; but what is true now may not be true by the time you get there.
