Are applicants typically keeping volunteer experience dating back to undergrad time for fellowships or only med school and beyond?

Not necessary. Nobody cares about what you did 10 years ago unless it's relevant to the specialty you are applying to. For example, if you did cardiology research as an undergrad and you are applying to a cardiology fellowship, then this will only strengthen your application.
