Do most schools require transcripts of volunteer hours? I've volunteered at various places where they didn't do any documentation.

Not most. Off the top of my head, ASDOH has a pretty thorough secondary that you complete online. They do ask for the volunteer places' contact information. I didn't find a log/transcript necessary though, because I had letters of rec that were pretty thorough.

Yes some do require transcripts of volunteer hours. Ohio state does for sure, as well I think VCU does as well. OHSU gives you this form to fill out, however it is very basic I would not stress out too much about it though.
