Do you use voluven? Why or why not? And if yes, under what circumstances? Thanks!

For me it’s kind of the easy button when I need a volume expander because it’s in our cart. I know there have been some outcome studies suggesting possible issues.

Well, sometimes SDN is good to tell you when you are an outlier. The black box warning is certainly ominous.

an old attending once told me, when they are bleeding blood, give them blood.

(Personally I think we’ve swung a bit to far with transfusion avoidance.)

Plenty of drugs have black box warnings like fluoroquinolones or droperidol. Doesn't mean we shouldn't use them.
 
In the private practice hospital we rotated through for OB we used voluven as a pre or co-load for c-section spinals to avoid hypotension/nausea. I probably only grabbed it ~70% of the time, gave a 500cc crystalloid bolus if I forgot. Couldn’t tell a huge difference, but that was their practice. See: Mercier, Curr Opin Anesthesiol 2012
 
what is voluven? I've heard of it but don't know much
