Do MD/PhD programs generally accept the letters from the AMCAS letter service, or do I have to send separate letters of recommendation when I submit the secondary for each school I submit a secondary to?

Of the schools I've applied to only one has ever not taken AMCAS letters and that's Duke. They use VirtualEvals I think and maybe they'll start using AMCAS in the future.

OK, thanks; will my recommenders need to send two letters to some programs (one through AMCAS, then one through AMCAS that is tailored for MD/PhD programs)?

No, each writer should send in only one letter.
