I think your profile is absolutely solid. Your community health worker experiences (intern and screening volunteer) is more non-clinical experience than clinical (pending description of responsibilities). This supplements your community hotline volunteering (description dependent). Metrics put you in an elite group of similar Hispanic/Latinx applicants; you have rockstar potential.

The challenge is articulating your purpose as a physician. If your goal is to stay close to Philadelphia for your relationship, look at the map and draw a 100-mile circle. Keep your in-state Virginia options, but you can aim higher IMO.
 
You have a good list and should receive several interviews. You could add any of these schools:
Harvard
Brown
Cornell
Emory
Vanderbilt
 
I appreciate all your help! I also wanted to ask about LMSA since I always thought it was for medical students. Should I obtain membership online first and then reach out to student officers through that platform? Additionally, would the purpose of contacting them mainly be to introduce myself and learn about how their schools engage with and serve Latinx/Hispanic communities?
The latter. Not sure if there is a premed membership rate with LMSA (while AMSA and SNMA might).
