Break down your religious non clinical volunteering so that the 125 hours of free food service is separate. Those 125 hours plus the 75 hours in the mobile food unit will put you over the 150 hours that many schools screen at. I suggest these schools with your stats:

Should I make it two activities or would it suffice to list it in the activity description?

I also have 300~ hr from high school that are continuous with my college religious volunteering. Should I include that or is it not helpful?
Usual rule of thumb is that we want hours that happened after you graduated high school. I would split your service to your religious community from service to the general community in need.
