I'm wondering if I have a good shot at academic programs in the middle atlantic, mountain, and pacific regions, all of which I have family ties.

Apply broadly. Make sure to include a healthy amount of places in the Midwest, as well as community places in your desired locations and also Midwest.

Unfortunately all of your desired locations are fairly competitive at baseline for DOs regardless of academic vs community, and the academic programs there can be fickle when it comes to inviting DOs. It can be hard to predict who will get invites and who won’t. Your chances are as good as they can be, but it still isn’t a guarantee unfortunately.
