Yeah, your list is top heavy and you have little to no service orientation experience to speak of. Examples include food distribution, shelter volunteer, job/tax preparation, legal support, transportation services, or housing rehabilitation. You should have at least 150 hours at submission to avoid getting screened out at most schools. Players in the higher-metrics pool should have 250 hours minimum at submission.
Profile wise, I can see how much you enjoy playing the trumpet. I don't see how you could let it go to pursue a medical career full-time (not completely let it go, but it's not clear you can be comfortable in uncomfortable situations with uncomfortable people without your trumpet). Compare your hours. You are not getting an offer because you play the trumpet though you seem to argue that as your "x-factor" you should be. "Education" is not a good theme here IMO.
