Did you get any interviews or offers? If so, who interviewed you and what was the outcome?
Are you a longtime member of the faith community you are serving?
DO admissions functions independently of MD admissions. But II's depend on your mission fit, not just your stats.
How many hours are you working with your family's business? What did you list last cycle? Is this the store clerk (1200 hours)?
Your in-states always give your best probability of an interview/offer. You got rejected, so there was something fundamentally challenging in your first app. What feedback have you receive to improve your application? I don't have a sense of why medicine is your professional goal from your description (original or revised), and that's enough to get you screened out. Apparently your two R's also suggest you don't interview well or fit with the programs.

Projected hours have minimal value so you should accumulate as many clinical, shadowing and non clinical volunteering hours as possible before you submit your application. I suggest these schools with your stats:
