Wanted: Case Log template for MS Access

I'd like to track my cases for my own purposes by using my own case log. Does anyone have such a log already developed that I could use to store my own data? I'm trying to avoid the programming aspect of MS Access.

Why don't you just make an Excel spreadsheet?

Surgical residents and attendings can log on to the ACS website now and log cases, but in the old days, for those of us more organized than to hand our program coordinator a shoebox full of stickers, we just put the data into an Excel file and onto our file on the hospital server so as to be able to edit it from anywhere.

When I was a 3rd year, I just kept a Moleskine notebook, put in a sticker for each patient, and wrote in the date, operation, and brief details of any part of it that I personally performed (i.e. close dura, close galea, close scarpa, subq or interrupted skin sutures, etc).

I still use the same method, only now I transfer the data into the ACGME's case log every week or so.

It's probably pretty anal of me to record which parts I actually performed. But it was very handy on occasion. A plastic surgery patient actually confronted me once, because she didn't like the way one of her scars looked and wanted to blame me for it. And I was able to pull out my little black book and point to exactly which scar was mine (not the one in question). Then she demanded to know who had done it, and although it was obvious to me who had, I was able to tell her truthfully that I only keep track of my own work.

That's EXACTLY what I do - I use a small lined Moleskine notebook to keep all my stickers. Uh...I don't keep track of which sutures I placed or anything though. 🙂

Halfway through MS3, i also started keeping a pocket journal. I document relevant details about the case, procedures, outcomes, treatments, notes. I also draw the out relevant MRI or CT findings and surgical approaches. I of course do this stuff within the bounds of HIPPAA guidlines.
