Western Prerequisites (BYU Students?)

I got my Western Prerequisite course sheet. I am a little bit confused as to which of my classes match the "Required Courses". At my school (BYU) all of the pre-dent students only take 1 semester of gen chem lab and 1 semester of o-chem lab. That is all they offer...But on the Prerequisites sheet there is a space for 3 different semesters of Chem Labs. There is also the option to check if the lab was included in the class.

BYU produces more pre-dent students than any other school; I know I have taken the right classes, I am just confused about how to fill the sheet out.

I feel like my post isnt making any sense unless you have seen the sheet. Is there anybody who has seen the sheet who might be able to help me? Did you guys end up leaving a lot of spaces blank? I am just worried that if I dont fill this form out right that I will be filtered out because it will look like I didnt fulfill the required courses.
 
Hey I'm predent at BYU also and I haven't applied anywhere yet, but I have often wondered about the lab pre-reqs. For most dental schools, it states 2 semesters of gen chem with lab and o chem with lab but all we can really take is chem 105/106 and 351/352 for the classes and then 107 and 353 for labs, which makes it seems a couple lab semesters short. If anyone else has any idea about this, I'm interested in hearing it too.
 
I'm also from BYU and just filled out Western's secondary. Chem 105 has the Lab incorporated into the class, so mark that box. Chem 106 has 107 as the lab. I marked Chem 353 down twice for the O-chem series and made a note that it is a two credit lab that counts for both the 351 and 352 classes. I'm sure they will get to know the way BYU structures its classes very soon.
 
I am positive we are not short! 2 semesters of gen chem is chem 105 and chem 106, plus lab is 107.
2 semesters of O Chem is 351 and 352 and the lab is 353. My question is not if we have taken enough classes. It is where do I put them on the sheet I am supposed to fill out...Has anybody seen this sheet? I need some major help!

Thanks Epictetus!!! Do you know if we HAVE to have 3 english classes? I thought that most schools only require 2 semesters...

And did you leave the spot blank for Chem and O-Chem semester 3? I cant help but feel like I am not fulfilling the requirements if I am leaving stuff blank...

I was wondering the same thing, but their requirement list says either 2 semesters or 3 quarters of English. The extra spot is just if you are from a school that does a quarter system instead of semester system. Same goes for the chemistries.
