With regard to what? Job security? Salaries? Problem patients? Job satisfaction? Intellectual stimulation?

The biggest problem I see is that physicians are simply getting burned out. Unless we see some significant changes that will improve their workflow, reduce their volume, and limit malpractice risk, we'll see more dissatisfied physicians.
