What are your challenges in clinical practice?

I am not sure if everyone will be able to answer this but I am looking for people who had a couple of years of experience in clinical work and are able to identify the challenges that they face with regard to paperwork, test results and patient information?

What would you like to see changed with regards to the above? I am currently working on a project to try and overcome some of the challenges of paperwork and am just interested in what it is that I should focus on.
