What constitutes a "gap in education"?

I have begun pre-writing secondaries, and a few programs include "explain any gaps in your education". Does this include a year off between undergrad and graduate school, or are they looking for people who took time off during college and came back to their education?
 
Mostly it means a gap... between HS and college, while in college, and, I suppose, between undergrad and grad school (why not?). What were you doing during the gap? You were working, recovering from an illness or injury, serving as a full-time volunteer, looking for work, etc.
It is frustrating when adcoms see a gap but have no idea what the applicant was doing during that time.
