What determines who gets fellowships after residency?

Im just a lowly 3rd year med student trying to figure out what determines if who get fellowships. I would like to do orthopaedic surgery and eventually do a fellowship in joint reconstruction at the end of residency, and Im trying to plan ahead when applying to residency, as I have heard the prestige of the program you go to may be a determining factor. If anyone could shed some light on what makes someone competitive for a fellowship after residency that would be great. Thanks
 
That may be true, but ortho's competitive enough that you'll be applying to a HUGE number of programs anyway, no?
 
If you manage to score an ortho residency, getting a fellowship afterwards shouldn't be a big deal. I would suggest maybe looking at places for residency that have the fellowship you're interested in. However, you'll need to apply broadly for residency and there are very few guarantees of placement in such a competitive field. Good luck
