What do adcoms ask when verifying your work/activities contacts?

I've been told that adcoms/medical schools calling/emailing your contacts for work and activities isn't as rare as I once thought, so I want to tell my contacts that there may be a chance that they will get contacted by medical schools.

Do they ask whether your listed hours are accurate or whether you were involved with the organization to begin with?

What would happen if your contact doesn't pick up the phone or reply back to the email (due to one reason or another)? Would this be a red flag?

If your contact does get contacted but their reply to the medical school is slightly different than the description you provided (due to different interpretations of the activities) (ex: if the contact isn't watching you every minute and may not know specifically what you are doing), will this be brought up in an interview so you can explain yourself or will they just red flag this and unofficially reject you?
 
We ask if they are familiar with the applicant and if they have an opinion regarding the quantity and quality of their work.
If nobody answers, we search the organization for another number.
Would you simply do an online search to find another organization member in that case?

What! I thought it was just to confirm dates and hours. Not a reference...

For some of the places I volunteer I don't even know who the volunteer coordinator is personally. At the hospital I currently volunteer at we've had 4 different coordinators over the past 2 years!

That's pretty common.
