What do you wish you had negotiated for?

I'm looking to start an inpatient position at either a major hospital system (non profit) or a stand alone psychiatric hospital (for profit) once I'm finished with residency and have now received preliminary offer sheets from both. For those who have gone through contract negotiations a time or two (or more) what are some things you wished you had negotiated for the first go round?

Patient cap, protected holidays, clarification of coverage. Everything is fine when all hands are on deck. Issues arise when people want off or when work loads become unreasonable when people are off. This is usually hidden in contracts.
