If a MD grad doesn’t get into a residency, what happens?

What are thier options?

Not matching isn't uncommon. Options usually include reapplying or taking a research year/clinical year to improve their CV.

You can certainly reapply. However, if you choose not to reapply or you continue to be unsuccessful in getting a residency, there are some great career opportunities for physicians without a residency. Your options are certainly more limited, but simply have an MD does carry some weight.
