I'm going into my 4th year so the plan is to graduate next May but I MIGHT be a few credits short and have to stay another semester. I want to make my PharmCAS account since it's more likely that I will graduate on time... however what happens if I don't and can't apply to pharmacy schools for an extra year?

You will have wasted your time and money and will not be able to attend pharmacy school. The vast majority of schools will not hold an acceptance for you to carry over to the next year.

Well if you at least have all of your pre-reqs done, you can still apply. I'm assuming you're missing credits to graduate with your bachelor's degree. In that case, it doesn't matter and you can still get into pharmacy school. You can also finish your degree while your in pharmacy school if your school lets you.
If you're missing classes from your pre-reqs and you don't think you have time to finish by May, see if the school allows you to finish by June or before matriculation. I know a couple of schools do that.
Either way, good luck.

Well if I apply for next school year and realize I didn't graduate on time or didn't finish my pre reqs, will PharmCas save everything (more specifically, my letters of recommendations) so all I have to do is resubmit?
Also, if I do graduate on time, and am just missing a pre req or two, will schools allow me to take it the summer before classes?

Pharmcas does not save any of your application for the following application cycle. You will need to re-do/re-enter everything: transcripts, grades, LoRs, personal statement, etc.

Some schools will accept summer classes (the summer directly before entering pharmacy school) to finish pre-reqs and others want classes completed the spring before entering pharmacy school. You'll have to research each school you're interested in to find their stance on it.
