Lets say, a woman comes to the clinic and demands a CA-125 to diagnose ovarian cancer which she thinks she has?

How should a doctor writing step 1 proceed?

Ask what her concerns may be regarding ovarian cancer. Ask what she knows about the utility of CA-125 as a diagnostic tool. Hard to say without multiple choice, but those are the two styles of answers I'd be looking for

Tell her you're sorry but it's too late :meanie:

👍

If there's an option exploring what the patient knows/thinks/understands, it will always be the right answer on Step.

But what would you do in real life?

I think asking them what they know about it is a perfectly reasonable starting point in real life, as well. It will help direct your response, help them feel heard, and probably help them to be more receptive to being told the screening test isn't a good idea.
