What is considered institutional action by AMCAS?

In freshman year a teacher gave me an F. Within a month I appealed and had my grade changed to a B+. During this month I was put on probation by my school. However since the grade was changed the probation was removed. I have gone to the registrars office at my school and they say that all I have on my records is a grade change.

Is there any reason to mention this as institutional action?
 
Since the probation was in error and was for GPA, I'd say no. The teacher sounds like a shmuck fwiw

If no institutional action shows up on your transcript or in your school's records, you do not have to report it to AMCAS.

Probably been asked 100 times but during my freshman year, alcohol was found in my room. My roommate and I had to take a drug alcohol course but that was it. My RA required it and such. Yes it was stupid and I am not going to put the blame on anyone else but I wasn't drinking. I should have stopped it from happening in my dorm I guess but oh well. I've since transferred from that institution and I haven't heard about it since. Should I just call the institution and ask if it is on my record? I honestly have no idea.

Yes, definitely call. AMCAS requires your transcripts from all colleges and universities you have attended. If the alcohol infraction shows up on your record and you fail to report it, this is a huge red flag. Something minor like this probably won't keep you from getting accepted to med school, but it WILL keep you out of med school if you don't own up to it.
 
Wow I'm going to feel stupid saying this haha. I forgot to mention that after the drug/alcohol course the instructor told us there would be nothing on our official transcripts. Is that enough of a reason not to report this?

Call the school anyway to make sure.
