What is MTM?

I've been doing a search for the differences between pharmacies, and noticed people saying MTM a lot. Nowhere did I see what it meant. People just kept using the abbreviation to it. When I searched for it, nothing popped up. So I googled it and found out it meant "Medication Therapy Management"

The definition being,

Can anyone give any more information, or word it better? I'm sure it is real simple, but it doesn't hurt to ask.

oo this is cool! i'm actually working on a research topic about medication review programs as innovative pharmacy services and this abbrev. popped up..anywho this is my attempt at the explanation

so basically, medication reviews, such as MTM, are opportunities for patients to consult the pharmacist (often in a community pharmacy) about the meds they're taking, if they're administering it the right way, if they know what the side effects are, interactions are, etc. Often the pharmacist will generate a complete medication record listing all the meds they're taking and additional information on how educated they are of their drugs. By allowing the pharmacist and patient get to sit down and have a one-on-one thorough discussion, it is hoped that patient compliance/adherence to their medications will be improved which is perhaps the most often anticipated result.

these kinds of services are considered "innovative" and "cognitive" because they are a great way of utilizing a pharmacist's specialization and improving the general public's awareness of a pharmacist's resourcefulness. similar programs have been conducted in australia ("home medicines review"), UK ("medicines use review"), and parts of canada ("medscheck") and the U.S. Very interesting topic..definitely read up more about it!
 
CVS started something called the Patient Care Initiative. Basically, we call up the patients when their refills are due, talk to them about their medication, and there is a lot more counseling going on when they are getting the medication. The pharmacists I work for hate it though, lol.

The term MTM or Medication Therapy Management is more popular now because of Medicare Part D. CMS or the Centers for Medicare and Medicaid have mandated that MTM services be available for patients with Part D by the year 2009. Other terms such as DTM or Drug Therapy Management and TDM or Therapeutic Drug Management are used as well. I work for a pharmacy association and am faced with these terms often. There is so much literature out there on this. I can probably direct you to so much more that's out there on this. This is taking pharmacists from behind the counter in the community pharmacy environment and in a clinical consultative position. It's the big talk amongst the pharmacy associations.
 
I would also add that MTM pharmacists can adjust the medications of a patient too - it's a lot more than just patient education. So if something is doesed incorrectly they can fix it and they can remove redundant medications. Some pharmacists have been given limited prescribing powers and can change medications to a different one in the same class if a patient is having issues with it. This type of pharmacy is becoming really important as people see more and more doctors and they don't check to see if anything is prescribed by someone else. Electronic medical records could help with this but that's not fully implemented yet.

mtm is a catch phrase they use to trick pre-pharm to think there's a future for pharmacy.
