Orbital decompressions are pretty brutal. I've seen a couple of exenterations - those would definitely be radical.
 
I think that a RON (radical optic neurotomy) is one of the most radical procedures. It is utilized when a pt. has vision loss secondary to CRVO(central retinal artery occlusion) that is refractory to treatment. The surgeon STABS the optic nerve/sheath with a specially designed scapel. I think that everyone in the O.R. hears the theme to Psycho during the procedure.

HE STABS THE OPTIC NERVE WITH A SCAPEL!!

It's actually called Optic Nerve SHEATH fenestration when done correctly, and Optic Nerve Fenestration when done incorrectly. Radial optic neurotomy is a different surgery altogether, where you jab a pointy object right into the optic nerve head via the pars plana.
 
Dr. Doan,

Since you trained at Iowa and I presume with Dr. Hayreh in Retina, I would be interested in your take on the RON procedure. From the research that I have done on this condition and treatment, Dr. Hayreh has quite vocal opinions about both. Just curious as to what is taught in your program.
