I'll be starting residency in July, and was wondering: what does the job market look like outside of the typical full-time, ~135 hr/month position at a conventional hospital/busy ED? For example, what options are available in the world of urgent care/rural medicine/international medicine?

On the interview trail, I heard several PGY-3's and 4's talking about pretty unconventional positions they had landed. One resident spoke about working 7 12's in a row in a remote location and taking the rest of the month off. Another was leaving to practice in the UAE for ridiculous amounts of money. A third told me he was going to be working 20 hours a week somewhere. Can one finish residency and work full-time, days-only at urgent care clinics and earn a livable wage? This is all word-of-mouth, but I was wondering how realistic these options would be.

I suspect I'll be looking for a pretty traditional position in a community setting, but some of these positions seem interesting. Obviously there would be huge pros and cons to each situation I described, but the temptation to live wherever I wanted and fly out somewhere one week per month is strong (while obviously terrifying to be doing single coverage 7 days in a row). Would love to hear what you guys think of these jobs.

You can work all days at an urgent care but you'll be way overtrained and likely get bored by the lack of acuity. You'll also take a pretty hefty pay cut. UCs tend to pay $85-120/hr.
