What to do after residency? Jobs and International Aid

I am a current PGY-2 and I planned on applying for fellowship during my PGY-3 year. This is all well and good except the fellowship that I'm applying for has a 2nd year application cycle, which means that I will have a year off in between residency and fellowship. My plan for this time is to take the boards in October then to use the rest of the time to participate in an international aid program. My biggest issue is finding some type of income from July until boards. Are per diem jobs worth it or will malpractice insurance bleed me dry? Has anyone heard of a 5 or 6 month contract with a hospital or a clinic? I'm sure my income-based loan repayment will be a mess, does anyone know how this will be affected? Does anyone that has worked in international health aid have any tips?
 
Sorry, no experience with international aid, but can comment on short term income. If you don't mind traveling, locums jobs may be your best bet. They usually set up licensing (and pay for it) and allow you to have an as needed or short contract. Malpractice should also be covered, but you need to make sure you have tail coverage when you leave. You'll need to contact a head hunter as opportunities often change quickly. Locums usually pays pretty well, so you'll probably have to start paying loans. Keep us updated in what you find (if you don't mind). Your experience will likely be valuable to others!

Edit: Also, start looking early, like June/July. you'll need plenty of time to make sure all credentialling and licensing goes through.
