Here are my testing patters.
224 UWSA1 on 3/26
238 UWSA2 on 4/15
226 NBME 7 on 5/1
217 NBME 12 on 5/6
231 NBME 11 on 5/11
221 NBME 6 on 5/15
Took the free 138 Qs(not sure if this is the free 150 everyone is talking about, but it was from the NBME site) and got an 84% correct. I took this sometime in the last 2 weeks, not sure when...
My test is on 5/23, one week from today and my goal is 230. Is that even possible now?
I've found my problem is not that I didn't recognize the diagnosis, it's that the answers are just something random.
Any help is appreciated. Thanks in advance!

I've found my problem is not that I didn't recognize the diagnosis, it's that the answers are just something random.

I've found myself to be the same way. I haven't taken the real deal yet, but what seems to help me on practice tests is this approach:
1. read the question quickly & come to a diagnosis (sometimes they'll tell you a diagnosis; sometimes the dx doesn't even matter)
2. read the question quickly again and figure out what, deep down, they're really asking. (maybe you can boil it down to "which of these enzymes is needed for aerobic glycolysis?", or "which of these cell types doesn't even have mitochondria?", for example)
3. read the question for details that will help you eliminate answer choices and eliminate any answer choices that have things in common with each other.

I don't know if what I just typed really makes sense outside of my head, but it helps to keep me from thinking too hard and/or confusing myself: just boil it down to the simplest version of the question you can.

i know the feeling.. sometimes i think they are trying to trick me and i overthink the question.. but they arent... hah.
