what to send along with the check

as checking the supplemental material page, it seems most school just ask for application fee and DAT score, since ADA sends the DAT scores, so I only have to give schools check. but what information should I put on the check to make sure they know who that money comes from? is name enough?

Usually they ask you to put your social security number on it. Although, I usually just added my name and AADSAS number to it.
 
If you want to be particularly anal and sure that the schools don't "lose" 😉 your money, attach a short letter with your full name, social security #, AADSAS #, amount of money, and check #. 😀
 
Never put your trust in the competence of a school's admissions office staffers. It's best to spell out everything for them in excruciating detail as if you're dealing with young children, and even then they can still find ways to screw up. So definitely write as detailed a letter as possible, but keep it short--remember, like children the admissions people have short attention spans.
 
I just put a little note in there that said Hello, my name is __________ and i'm applying to your dental school this cycle, enclosed is my check for $_____. then on the check i just put "application fee for" and my full name on the memo line. i haven't seen directions anywhere for what you are supposed to put on the check, i haven't recieved my aadsas # yet, and didn't really feel comfortable putting my soc. number on there. with my full name on the check and the note, i would think they would be able to figure it out since nowhere does it give instructions for what to include on the check.
