When an Attending offers to write a LOR, do I have them send it to me directly?

Do I have them mail it to my school?

Do I give them a SASE?

If you need it for this year's application, you need the cover letter signed by you with a return address of your school's ERAS office. If you need it for next year or even later and your professor plans to stay in town, you should ask them to make notes now and write it closer to when you are going to apply. That way you can give the most up to date CV, personal statement, as well as know what specialty to which you are going to apply. Letter writers who offer to write the letter (rather than accept when you ask), will personalize it and tailor it towards your specialty and or geographical preferences.

That is great advice.

I wasn't sure how that worked or if I should offer to write the letter.

It happened today - my first thought was to send a quick note to the Attending's practice (my rotation ends Friday) but then, I didn't know what to say and I probably won't see them as a rotating student again.

I just started my 3rd year, so it will be year before I need the letter.

If you wont need it for another year I would have them send one to the school and one to you (if they are willing).

What you can do is when you are getting ready to apply to residencies dig up the letter and meet with the attending to see if he/she will "update" it for you. New date, maybe a few more nice comments, etc.
