yes...it wouldn't make sense the other way (some would argue that the match process doesn't make sense, but that's another topic). in addition, most programs do not start ranking applicants (at least in IM) until after the interview season is over.

Yes.

The rank order list is due sometime in mid to late February.

You can't submit a list if you didn't interview any place.
