When do you call the dr to change a dose?

If you get an amoxicillin rx for a kid and the dose should be 1200mg but it comes out as 1300mg do you call. Do you call if it's anything over the maximum, say 1250mg. or only if it's grossly over. I only ask because every pharmacist i ask seems to only bother if it's grossly over. But what would be considered the begining of being grossly over, 10%too much?
 
I only call if it's ridiculous. A dose of 1300 isn't going to give the kid seizures. Given that the high dose for Amoxil is 90mg/kg, that extra 50mg is analogous to 0.5kg and is within reason. But that's just me.

Technically you are supposed to call if it's over at all though and I'm probably going to make a horrible pharmacist - so keep all that in mind.
 
Call if it is over and document. Cover your ass always.
 
Agree with others - the antibiotic doses for kids can get really high due to local resistance patterns. Also....before you call...make sure you really know the kid's weight - you might be surprised!

The cough syrups are worse than the antibiotics for doses that get too high.
