When to apply to elective?

So we get to do an elective at the end of our third year, in May or so.

When should I be contacting programs and applying?

And what's the process like? Do spots fill up quickly?

From what I've found, most only accept fourth year students.

Most of the applications go out in February-March of your 3rd year. I would take a look at them before hand though, as some have requirements which will take some work on your part (LORs, excessive amounts of blood work, etc.). If you're able to rotate in May, you may have to put in your application earlier depending on the program (November/December?).
