Where to take OAT?

I'm sorry if this question has been asked before but I'm graduating from college this year and I really need to take it this summer. I need a location for a testing center in AZ if anyone could please let me know how I can go about setting up a test date to take the OAT and when is the last day of testing is would be also be helpful.
