If I am not misinformed, even Penn has top 10 ranking, so even an ivy, specialty focused schools have some form of truncated ranking system.

so I was wondering, does all schools have some form of class rank?

I don't remember which one it was, but either NYU or Columbia doesn't rank.

Most schools are switching to a rank form because the boards are now pass/fail. They need to differentiate students some how.

UCSF doesn't give out rankings to residencies but they (professors/whoever) do keep track of performances and in their letter of recommendation, they write "highly recommended", "recommended", etc.
