Which institution affiliation to use for journal article

Hey guys, I've been trying to google this question but couldn't find a totally relevant answer...

If I am writing up a case report about a patient I saw while on an away rotation, which institution should I put for me? My medical school or the school I did the away rotation at? I am the one actually submitting the article as well which requires all this corresponding contact info and it's not like I have a departmental address/fax/etc.

I've had a similar situation, use your affiliation (meaning your native med school). Look at journal submission guidelines carefully, some journals require an IRB or an IRB waiver for case reports (could be complicated corresponding with the other institution if that is the case).

Bottom line to your question: use your med school not the away.
