which schools offer grade replacement?

Hey does anyone know where there is a list of pharm schools that do grade replacement? preferably ones in the midwest but a complete list would be quite helpful too. Ive looked all over for this info and it seems pretty elusive for pharmacy..

USN does grade forgiveness, meaning you have the option of only counting classes taken within the last five years, but all of your prerequistes have to be taken within that time period. Xavier also takes only the highest grade for a class. Those are the only few that I know of at the moment.

Last time I checked, Oklahoma, UMKC, Tennessee takes the average of the repeat courses, Utah (takes 2nd grade only), Colorado (I think). Most schools want the pre-reqs within 5 years, so that can also be considered.
