I read your other thread in the DAT section and think that it's going to be an uphill battle (but you already seem to know that). Most, if not all, require a processing fee. However, I'm guessing your question is probing for the schools that have additional essays for you to fill out. Some schools (e.g. Creighton and Penn) have additional essays that you can look up online anytime. Others screen your application before sending you a supplemental application (e.g. UCSF, UCLA). Others only have a supplemental application when you interview (e.g. NYU). And others just don't have applications, just send money (e.g. Columbia, UoP). Depends on the school; just read the AADSAS directions and each school's website for more details.
I hate to be the bearer of bad news, but I sort of agree with bluesprite and that the 13 in QR is going to be an issue and would go so far as to say that it will close a lot of doors (as some schools have stated minimums; could be per section or overall - I suppose that depends on the school). And given a lower science GPA, you really needed a high DAT (both AA and TS) score to help your case out. Since you are applying next cycle, make sure you have an incredibly polished personal statement and gather extraordinary letters of recommendation from science professors/dentists because you have a lot of ground to make up.
Not really sure how to approach picking schools if I were in your shoes. I probably wouldn't bother with Columbia, given the prevailing idea that they prefer higher DAT. I just don't see this being a viable "long shot." I likely wouldn't try for out of state public schools either, since those spots tend to be a little more competitive. I would likely focus on my public in-state schools and privates. Who knows, maybe with your experience, schools like ASDOH and the Midwesterns could be really viable options.
