Which SOAP offer would you take/.

1. Prelim IM year at a large community hospital. This hospital is loosely affiliated with a large academic center. The community hospital does not have your specialty of choice to enter PGY-2, but the academic center does.

2. Categorical spot in your backup specialty at a large academic center. This place has every specialty/fellowship known to man.
 
I'd take #2, there is no guarantee you will match into your specialty of choice next year at least with the categorical spot you know have a sure thing. Nice thing is It is in your backup specialty so obviously something you considered doing. Maybe you will love it and not want to reapply.

I agree with whats been said. If you can see yourself happy in that specialty and in that place then take it. You can relax knowing you don't have to match anymore and devote yourself to being happy in your new home.
 
Unless #2 is horribly malignant and you don't think you could handle the work hours/environment there, then you definitely want to go there. As someone who is practicing medicine now, it matters very little where you did your residency at and even if it was deficient in certain areas it's very easy to bridge those gaps once you're out in practice. You want to get through residency as quickly as possible and move on with your life. If you have a residency in hand, just take and go with it, make the best of it.

Why woud a PD feel the need to say "we would love to have you here in July" and "you are an excellent candidate", during SOAP? It's not like we get the chance to rank them.

Because if you were to get >1 offer including theirs, they want you to pick them.

Plus, we get so used to saying it, we can't help it.
