Which time can we use bacteriocidal drug or bacteriostatic drug ?

If we give a patient who has negative bacteria infection a bacteriosidal drug --> bacteria death --> more endotoxin will be released --> the bacteria will harm the patient more 🙁
so can we only use bacteriocidal for positive bacteria to avoid endotoxin affect 😕

Most of the times I've seen when they're concerned about bacteriostatic vs bactericidal involve cerebral abscesses and bacterial endocarditis, in which case bactericidal agents are considered superior for treatment.

Endotoxin can certainly cause problems, but treatment of syphilis is done with penicillin (a bactericidal agent) barring allergies and it is famous for causing Herxheimer reactions which are caused by endotoxin release.

The endotoxin is there already. How long do bacteria live anyway before dying? Not long, and in the meantime are producing MORE of themselves. So, it's best to kill them.

Now both bacterial -"cidal" and -"static" drugs will kill the bugs, but we like cidal bugs for more serious infections, as has been previously mentioned, like meningitis and endocarditis.

This is yet another reason cultures are very important. Vancomycin is a bacteriostatic drug, and is actually a pretty poor antibiotic but it's the most cost effective way to treat MRSA, so that's why you see almost everyone on it. Nafcillin is a great drug but obviously only effective in MSSA. So it's great when the culture is MSSA because you can drop the vanc and add nafcillin and get much better results.
