Who should I put as EC contacts for these activities?

Hello! I was just looking for some feedback on who to put as EC contacts for two activities.

1) I worked as a scribe at a private practice but resigned due to problems with the administration. I'm afraid if I put down the practice manager as my reference, he'll badmouth me. For the EC contact, does it need to be a boss, or can I just put down any other employee that I worked with who can verify that I was actually there?

2) I was on board of directors for a club in freshman and sophomore year. It's been over one year since I graduated, and I am no longer in touch with any of the other leadership. I have literally no contact information for them. It was completely student-run so there is no faculty adviser or anything. Since I don't have any contact info, should I just leave off this activity?

1) A co-employee is fine. They wouldn't be asked for a reference, just a confirmation of timeframe of involvement/~work hours.

2) You can use yourself for a Contact.
