I didn't switch into EM, but one of my fellow residents did. He said that after a year of IM, that he realized that he still couldn't save someone's life. He could do great notes, take very thorough histories, but when a patient was crashing, he felt very inadequate in preventing or reversing their death. So he chose ER, to be able to make more of an immediate difference.
