Why do some schools ask for more info on institutional actions?

Why do some schools ask for more information/certification if you indicated you had a minor institutional action (alcohol/weed freshman year incidents) when the vast majority of schools trust your word as an applicant? I am confused... do these schools just not trust you, even after you demonstrate integrity by informing them of an IA? Or do they just want an official statement for due diligence? Keep in mind the vast majority of schools take your word as an applicant and do not request further info, but a handful of schools ask for a statement from your undergrad, etc.

They're doing what the legal dep't tells them what to do.
